The Windsor Connection Resource Center

The Windsor Connection Resource Center provides a single home location for a variety of community service organizations and individual practitioners who, working separately and together, provide a wide range of health and human services to members of the local community. MAPP is one of these service organizations. The resident providers at WCRC work together in an alliance called the PATCH Team, founded with the express goal of bringing vital services, education, and enrichment opportunities to all community members through:

  • information
  • referral
  • advocacy
  • case management services

The PATCH Team structure facilitates collaboration among practitioners, resulting in better coordination of services and a more stable service environment for the team's clients. In this way, PATCH helps the WACP build a positive sense of community.
